Prescriber Identity Proofing

Part 1: Credit Card Validation

To verify an individual prescriber, Cirrus Medical Network will collect the individual provider's credit card number and, after a 2-3 day waiting period, create a nominal charge which will be refunded.

To start the process, please complete the Paypal/credit card purchase, then come back to this page to verify the charge:

Click on the "Verify by Credit Card" button to report the nominal charge on your credit card statement.

Step 2: Credential Issuance by Mail

The second step of identity verification is via mail. Cirrus Medical Network will mail a PIN# to the individual prescriber's home address of record (e.g. credit agency; governmental agency, if permitted; Verizon; Experian; Equifax' or other non-public source).

Credentials cannot be issued to a phone number of record, an e-mail address, or otherwise provided electronically.

Fill out the following form to have a letter with your PIN# sent to your home of record by mail:

Submit "home of record" address.

    We will mail a PIN# to your home address of record. Home of record addresses are used by credit agencies, governmental agencies (if permitted), Verizon/ATT, Experian, Equifax, and other non-public sources. Credentials cannot be issued to a phone number of record or an email address.

Upon receipt of the PIN# in the mail, you may submit that number electronically by clicking on the button below "Submit my PIN"
